The “flash sideways” was some sort of limbo that existed for the dead Losties some time in the future – presumably after Hurley was done protecting the Island (may have been thousands of years). Once enough of them resolved their issues they were able to pull away from the limbo and prepare to move on together to whatever lies next.

The Losties lived on the Island throughout all the seasons and everything we saw, happened. The FST was the afterlife that the Losties went to once they died. Even if they died when they were 90 years old, they went to the Afterlife as the age they were when they were on the Island.
